On this page you will see a number of downloadable forms and Help Guide including a 'Supplier Assessment Form'. Please download, save and print all documents provided.

You should complete the Supplier Assessment Form for all existing and new suppliers who supply you any ingredients and / or ready to use food products. This enables you to maintain a record of your suppliers and provide a simple monitoring system to demonstrate 'due diligence' when sourcing food products.

You will also see that a further monitoring form has been provided to support the gathering of appropriate supplier information for your FSMS. A third downloadable control form also included is a central list allowing you to list all the suppliers you deal with in one form.

Reputable suppliers should easily be able to supply you with the information you require to quickly complete these forms. These forms are pre-requisites and 'essentials' that must be in place to help set up your FSMS.

Delivery Checking - Every delivery should be checked for condition of products, temperature level and packaging. The form should be completed for every delivery.

The final download included is 'Help Guide 1 - Suppliers and delivery'. We have provided this as extra information to support you, so any additional questions or concerns you may have on suppliers and deliveries will probably be answered by this guide. In common with all the Help Guides provided we recommend that you download, print and store it in your FSMS file in order for you to quickly and easily refer to it when the need arises.
